What You Need to Bring!

Only TEFAP USDA food may be provided to clients without requested documents at each visit.

Photo ID for the Person Requesting a Food Pantry.

Any form of government issued photo ID is acceptable.
Please bring every time you come if you desire to receive our full selection of food items.

A current piece of mail or payment receipt verifying your address.

Proxy Consent Form Required for individuals unable to pick up food from Project Hope in person.

Mail or receipt should include your full address dated within the last 30 days.
Mail to the current resident is not acceptable.
Please bring every time you come if you desire to receive our full selection of food items.

A Proxy Consent Form must be completed for each person authorized to pick up food.
Beneficiary Name and Address is the person needing the food.
Name of Proxy is the person picking up the food.
Form must be signed and dated by the Beneficiary.
Bring Beneficiary Photo ID and Current Piece of Mail to obtain our full selection of food items.
